In other words, the British jihadist wants to kill David Cameron or to see him punished in some other terrible way.
Head on a spike as an expression is probably inspired by people who primitive wars in early human history. Back then, people sometimes cut off the head of an enemy soldier and put it on a spike, a long sharp nail such as the top of a spear – and display it as a trophy.
In our example, Cameron’s fellow countryman wants to see him severely punished or humiliated for "waging war" against Muslims.
